| Description |
The program is designed to improve the overall quality of care for eligible residents of adult care homes. Eligible residents are seriously impaired and require extensive assistance in the areas of ambulation, toileting and eating. Residents receive additional personal care services from adult care home staff in these areas and comprehensive case managmenent services from the county social services staff. Needs are identified through assesments and a service plans are developed, implemented and monitored as long as the resident meets the eligibility criteria. |
