| Description |
Aging and Disability Resource Centers (ADRC)have been developed in Forsyth and Surry counties. The Centers help individuals and families make informed decisions about service options and serve as the entry point to the long-term care system. They do this by strengthening and improving the linkages among existing 'points of entry' through a 'no wrong door' approach. This allows consumers to receive the same information and ease of access to services regardless of how they enter the system. Each ADRC is required to provide public awareness, assistance and access to long-term care services. Other activities and products include: support the implementation of NC Care Link, a statewide information and assistance web portal on community resources; expanded use of volunteer benefits counselors; implementation of simplified Adult Medicaid mail-in application; pilot of a uniform tool for client intake and screening;and coordination with a high cost chronic care management program. |
