| Description |
The program serves Mediciad eligible disabled adults who are determined to be at risk for abuse, neglect or exploitation. Case managers assess the needs, and then plan, arrange, coordiante and monitor services provided to eligible indiviudals and families. Efforts are made to acceess less costly home and community based services and prevent situations that would require care in acute or long term care settings. |
