| Description |
Provided by county departments of social services, case Management and counseling includes an array of services designed to meet the individual's needs. Included are Health Support Services, Personal and Family Counseling, Individual and Family Adjustment Services, Representative Payee Services, Employment and Training Services, and Case Management Services. Assessments involving all realms of functioning (e.g.,environement, mental/emotional health, social, physical health, economic) are completed, service plans are developed and services are provided or arranged to strenthen functioning in those areas where limitaions or needs are identified. Quarterly reviews and annual reassessments are completed to monitor progress, and update service plans as needed. |
