Description The Chronic Disease Program works to develop healthy, safe communities and health systems to prevent and control chronic diseases and to eliminate health disparities. This is accomplished through policy development and environmental changes that support healthy behavior and improvements in systems of care as well as through education, screening, direct medical service, community engagement and media. Emphasis is placed on collaboration and, when feasible, integration of work among programs to achieve common goals. The Program includes the following chronic diseases: asthma, cancer, heart disease and stroke, diabetes, renal disease, and epilepsy.
