| Description |
The Independent Living Rehabilitation Program provides options for persons with significant disabilities to achieve independence to the greatest extent possible to reside in the homes and communities of their choice, and to avoid the need for managed care facility-based living as long as possible. The program is eligibility based with a primary focus on people who may be able to manage their lives and live independently, but whose functional limitations will not facilitate independence without services such as: an adapted environment, assistive devices for mobility, lifting and communication, and/or part-time personal assistance (consumer managed) to conduct activities of daily living. |
