| Description |
The Division awards funding to county departments of social services for contracting with local adult day service provders for adult day care, adult day health or combination services. Adult Day Care and Day Health programs provide an organized program of services during the day in a community group setting in order to support independence and promote social, physical, and emotional well-being. Providers are reimbursed at the daily rate of up to $33.07 for adult day care and $40.00 for adult day health care. Providers have the option of providing transportation services. Reimbursement for transportation is $1.50 for a one-way trip and $3.00 for a round trip. |
